By Michael Harris It simply amazes me. Everyday I see otherwise smart individuals cramming garbage into their faces without a single thought about what it’s doing to their body. They go out of their way to keep their vehicle in top running condition by putting in the best gas and engine additives (to keep the fuel lines clean and prevent gunk buildup), put in fresh plugs and wires (gotta have decent pickup!) and wash it every week. I can completely understand this attitude because a car’s a major investment and you want to keep it as long as possible. On the other hand, your body is yours for life! You only get one. So why would you treat your body with any less respect than you treat your vehicle? Why wouldn’t you want to put only the best fuel available into your personal “machine” to keep it in optimal running condition? By eating junk food and drinking sugary sodas and fruit drinks you’re basically telling yourself that you don’t matter. That your material possessions are more important than your own health and well being. Doesn’t it make sense to give yourself the best possible chance at staying healthy when you’re the one paying the bills? I have a friend who’s a fairly shrewd business man. He sells cars on the side to make extra cash and he always makes a profit. He knows a lot about cars and always keeps his own vehicles immaculate inside and out. He’s also risen to the top of his chosen profession of printing and has a good family life. He’s a winner. Physically, on the other hand, he’s a different story. He has high blood pressure which causes his hands to shake uncontrollably. His family has a history of heart disease. So what does he choose to eat when it comes time for his lunch? A candy bar, a muffin and a 20oz. bottle of Mountain Dew! Wow! The reason I bring up this particular friend (because I could use many others as an example of poor diet) is that he recently had a bout with hemmorhoids which caused him to alter his diet for the better which included cutting out the sugary sweets and soda and eating some roughage (Wasa Crisp crackers was the choice here), more raw veggies and drinking water instead of soda. This is the premise behind a true low carb diet plan. Cut out the simple carbs (sugar and processed foods) and eat sensibly. So what did this drastic dietary change result in? An amazing transformation to say the least. His hands almost stopped shaking completely (lowered his blood pressure), his energy increased (no more sugar spikes and crashes) and he just all around felt better. Oh, and his hemorrhoids got better too! Now, you’d think that with all these nearly miraculous turns in his overall health, he’d stick to what’s working and reduce his risks of heart disease and other ailments. Nope! As soon as he felt better he went back to his old diet (if that’s what you want to call it!) and back came the shakes. I doubt that everyone with bad dietary habits shows such obvious signs of their effects as my friend does, but rest assured that your diet is directly affecting your health. Depression Series: Why Don’t I Respond to Medications? (Part 1)

By Michael Rayel Maria has been feeling depressed for at least two and a half years. About three years ago, her husband of 20 years left her for another woman. Devastated, she became despondent and tearful almost daily. Eventually, her depression got worse associated with inability to function. Her appetite, energy, concentration, and sleep became impaired. She also felt hopeless and suicidal. Her psychiatrist put her on a starting dose of antidepressant. She responded initially but after a few days, she felt just like before taking the medication. For the past two years, Maria has tried four types of antidepressants. She has taken the usual adult doses of these drugs. Although she somewhat improves, she has virtually remained the same depressed and disabled. Maria seems to be taking the medications regularly. But why is she not responding to her antidepressants? Maria is just one of the many depressed individuals who dont feel normal despite treatment. Depression is a treatable disease but how come some people dont do well on medications? There are many reasons why depressed patients like Maria dont improve on antidepressants. First, is the diagnosis correct? Depression can be caused by many clinical entities. Sometimes, knowing the right diagnosis is a challenge. Medical disorders, medications such as beta-blockers and benzodiazepines (e.g. clonazepam), and various psychiatric disorders can cause depression and they all require different treatment. If your doctor fails to identify and treat the true cause of your depression, you will remain depressed despite the use of antidepressant. Second, are there co-morbid disorders? Depression can exist along with other psychiatric disorders such as anxiety disorder, alcohol or drug problems, personality disorder, dementia, and psychosis. Depression will persist if these co-morbid disorders are not treated. For instance, depressive disorder with psychosis cannot be adequately treated just with antidepressant alone. You need an antipsychotic drug added to an antidepressant to treat the illness. Third, is there an ongoing neurological or medical disorder that precipitates, aggravates, or complicates depression? Hypothyroidism, hyperthyroidism, vitamin B-12 deficiency, pancreatic cancer, brain tumor, Parkinsons disease, and stroke can all cause depression. If any of these disorders are present, antidepressants are less likely to help. The goal in these situations is to treat the underlying medical condition. A 65 year-old lady came to see me complaining of severe depression. On evaluation, she disclosed that she had been on three types of antidepressants for the past four years with minimal response. I checked her recent laboratory results which showed an abnormal thyroid! No wonder, she was not responding to the medication. Fourth, are there ongoing psychosocial issues? Financial problems, family conflict, work-related stress can all precipitate and complicate depression. Despite adequate medication treatment, some individuals will remain depressed especially if such problems are not addressed by the therapist or psychiatrist. Is there any way you can reduce the stressors? Please do so the earliest you can. The treatment of depression is frequently straightforward. Occasionally however, various factors complicate it. For antidepressant to be effective, a psychiatrist should ensure that the diagnosis is correct, that co-morbid psychiatric disorders and medical problems are treated, and that psychosocial issues are adequately addressed. Root Canal – Your Dentist or an Endodontist?

Do you let him or her perform the procedure or do you need an Endodontist, root canal specialist? Before going any further, I need to let you know that I have worked for an Endodontist for over eight years, so my advice will be biased toward Endodontists. But I have my reasons, as I will explain. What created the need for Endodontists in the first place? 1. A toothache is an emergency that can be caused by a broken tooth or abscess. This patient will need treatment immediately to control the pain, but your dentist has a lot of patients and books a full schedule every day. He can refer this patient to an Endodontist whose schedule is usually more accommodating and know that the patient will be treated correctly. A caring dentist knows that he/she cannot possibly accommodate all emergencies. They develop a good working relationship with a local Endodontist as a way to offer their patients immediate care. An Endodontist can also be a back-up source for the dentist for vacation days, etc. 2. Root canals can take a lot of time for a dentist. This is a big point so listen carefully. An Endodontist does a lot of root canals and has the procedure down pat. For most patients, the less time spent in the dental chair, the better. My Endodontist can do a molar root canal from start to finish in less than an hour for a normal tooth. 3. Note, I said ‘normal’? Teeth can have some funky configurations. Roots can curl around each other, canals can be calcified, or a tooth may have more canals than normal. An Endodontist has seen enough teeth to know what to look for and handle any abnormalities your tooth may have, and in a reasonable amount of time. For example: a. I have known patients that have been in their dentist’s chair for hours for a root canal. I remember one patient that claimed to be in her dentist’s chair for 8 hours, and still ended up at the Endodontist for completion of the root canal. b. I have known many patients that were referred to an Endodontist because the dentist accidentally perforated the root trying to do a root canal. A perforation is when the dental file punches a hole through the outside of the root. That’s not to say that an Endodontist never makes this mistake, but it’s a whole lot less likely since they handle these files day in and day out. They also have expertise and special materials for repair of perforations. c. I have know many many patients that had extra canals that the general dentist did not see or treat causing the tooth to abscess afterward creating the need for a retreatment. d. I have known patients that had two of the canals treated by their dentist only to find the third canal calcified, and had to be referred to an Endodontist to finish the root canal. This is frustrating for the patient, to put it mildly. When a dentist starts a root canal and cannot finish it, legally this tooth is considered a re-treatment. Retreatments are more expensive than a regular root canal because of the specialized equipment, time and expertise required. Also, because the Endodontist is treating a tooth that someone else has worked on, he is, in essence, accepting legal responsibility for someone else’s work. Sometimes, that work needs to be corrected and it takes more time for a re-do than to do the root canal in the first place. Patients do not understand this and get angry that they have to pay, not only twice, but more for the second procedure. And some dental insurance will not pay for a retreatment immediately after the original root canal. These examples are not the norm, but they are seen on almost a daily basis at an Endodontist office. Of the millions of root canals done daily, most dentists perform beautifully with no problems. But if you see your dentist looking doubtful when he looks at the x-ray of your problem tooth, and he recommends an Endodontist for your treatment, please follow his advice. As for flowers around or on the altar, it is imperative that the instructions for decorations are clear. Some churches will not allow any flowers or decorations on or near the altar. Another point to keep in mind is what the plans are for the flowers after the wedding. Some churches will appreciate a donation of some flower arrangements while others may not. When planning a wedding the flowers are a very important part and very time consuming. From the decision making to the actual wedding, flowers may consume a lot of the planning time, however they are a special piece of the wedding and you will treasure the memories for years to come. Fewer Complaints About The Signal Strength

All four providers will tell you that they have the fewest complaints and several have done “independent” surveys that they pass off as unbiased, but since they pay for the surveys, they are rarely unbiased. In general, I would have to say that Verizon has the least amount of customer complaints, and if you only need to use your phone in the US and their coverage is good in your area, then they are a good choice. If you need to be able to take your phone abroad, then I would suggest Cingular. Currently, Cingular has the largest US footprint (since the merger of AT&T Wireless) and the most roaming partners internationally. Because Cingular has a 30 day return policy, you could also try them and see if your experience improves. They are GSM, like T-Mobile, so if GSM is the issue in your area, then you may not have a better experience. Verizon and Sprint have 15 day return policies, so you can also try them without worry, just for a shorter time. I think you are going to be able to tell right away. I live in the country and use a Palm Treo from Cingular. I cannot get a signal unless I go to a top of a hill on my property. I have a cellular antenna on my car that works when I am on the road but is pretty useless on my property. Is there a way I can put an antenna on my house. Your best chance for gaining a signal in your house will be realized with a Yagi antenna on your roof with a cellular amplifier in the antenna line to boost the signal. The Yagi antenna is a directional antenna that you aim at the tower. There are two Yagi antennae for cellular. One is for the 800 MHz band and the other is for the 1900 MHz band.
